Overview: Avmapki Fakzynja Co-Pack is used to treat ovarian cancer in certain people. It contains two medicines that work together to slow the growth of cancer cells. The most common side effects are nausea, tiredness, upset stomach, skin rash or acne, and changes in lab tests. Tell your health care provider right away if you have sudden vision changes or eye pain, blisters or other skin reactions that are getting worse, yellowing of your skin or eyes, or unexplained muscle pain. Avmapki Fakzynja Co-Pack comes as capsules and tablets that you will take by mouth.

Avmapki Fakzynja Co-Pack (avutometinib and defactinib) is commonly used for a certain type of ovarian cancer.

Avmapki Fakzynja Co-Pack may also be used for other conditions as determined by your health care provider.

How does Avmapki Fakzynja Co-Pack work (mechanism of action)?

Avmapki Fakzynja Co-Pack contains two medicines: avutometinib and defactinib. Avutometinib blocks a protein called MEK1. This protein helps regulate signals inside the cancer cell that tell it to grow and divide. Defactinib blocks two different proteins that affect a different set of signals that help cancer cells survive and spread. By blocking different proteins, these medicines work together to slow the growth of cancer.

What are the serious side effects of Avmapki Fakzynja Co-Pack?

While less common, the most serious side effects of Avmapki Fakzynja Co-Pack are described below, along with what to do if they happen.

Severe Allergic Reactions. Avmapki Fakzynja Co-Pack may cause allergic reactions, including a specific type of allergic reaction called drug reaction with eosinophilia and systemic symptoms (DRESS). It is also called multi-organ hypersensitivity. This is a reaction that can affect multiple parts of the body including your liver, kidneys, and heart. Stop taking the medicine and get help right away if you have any of the following symptoms of a serious allergic reaction.

  • Breathing problems or wheezing
  • Racing heart
  • Itching, skin rash, or pale red bumps on the skin called hives
  • Fever or general ill feeling
  • Swollen lymph nodes
  • Swelling of the face, lips, mouth, tongue, or throat
  • Shortness of breath, trouble swallowing, or throat tightness
  • Swelling in your feet, ankles, or legs
  • Nausea or vomiting
  • Dizziness, feeling lightheaded, or fainting
  • Stomach cramps
  • Joint pain
  • Dark-colored pee
  • Your skin or the whites of your eyes turning yellowish in color (also called jaundice)

Eye Problems. Avmapki Fakzynja Co-Pack may cause serious eye problems, such as an inflamed cornea (keratitis), retinal vein occlusion, or retinal detachment. Keep all appointments to have your eyes checked before and during treatment with Avmapki Fakzynja Co-Pack. Tell your health care provider or see your eye doctor right away if you have new or worsening vision problems like any of the following symptoms.

  • Blurred vision or double vision
  • Vision loss
  • Eye pain 
  • Conjunctivitis
  • Dry eye
  • Seeing flashes of light or spots that float across your vision
  • Photophobia

Severe Skin Reactions. Avmapki Fakzynja Co-Pack can cause severe skin reactions, including erythema multiforme (EM), and acute generalized exanthematous pustulosis (AGEP). Your health care provider may give you certain medicines to help lessen skin-related side effects while taking this medicine. Use these medicines exactly as directed. Tell your health care provider right away if you have new or worsening skin problems, such as any of the following. 

  • Rash 
  • Blistering of the skin or mouth
  • Mouth sores
  • Skin peeling or infection
  • Dry or itchy skin
  • Acne
  • Redness or swelling of your face, hands, or soles of your feet 

Liver Damage. Liver damage, also called hepatotoxicity, can happen when taking Avmapki Fakzynja Co-Pack. Keep all appointments to have your blood checked. Call your health care provider right away if you have any of the following symptoms of liver damage.

  • Nausea or vomiting
  • Stomach or belly pain
  • Fever
  • Weakness or unusual tiredness
  • Itching
  • Loss of appetite
  • Light-colored poop
  • Dark-colored urine
  • Your skin or the whites of your eyes turning yellowish in color (also called jaundice)
  • Easy bleeding or bruising

Muscle Problems. Avmapki Fakzynja Co-Pack may cause muscle problems. Muscle problems may be found by an increase of a protein in your blood called creatine phosphokinase. Keep all appointments to have your blood checked. In some people, the muscle may break down (rhabdomyolysis), which can be serious and rarely lead to kidney damage and death. Get help right away if you have any of the following symptoms of rhabdomyolysis.

  • Unexplained muscle pain or weakness that does not go away
  • Trouble moving your arms and legs
  • Low energy levels or feeling easily tired, especially with activity
  • Dark, red-colored urine (pee), or peeling less than usual
  • Fever
  • Muscle cramps
  • Stomach pain
  • Nausea or vomiting

Warnings & Precautions

Who should not take Avmapki Fakzynja Co-Pack?

Allergies to Ingredients. People who are allergic to any of the following should not take Avmapki Fakzynja Co-Pack.

  • Avmapki
  • Avutometinib
  • Fakzynja
  • Defactinib
  • Any of the ingredients in the specific product dispensed

What should I know about Avmapki Fakzynja Co-Pack before taking it?

Do not take Avmapki Fakzynja Co-Pack unless it has been prescribed to you by a health care provider. Take it as prescribed.

Do not share Avmapki Fakzynja Co-Pack with other people, even if they have the same condition as you. It may harm them.

Keep Avmapki Fakzynja Co-Pack out of the reach of children. Since this medicine should be kept in a refrigerator, take special precautions to keep it away from children who also use the refrigerator.

Take Avmapki Fakzynja Co-Pack with food. Each dose will have multiple Avmapki capsules and a Fakynja tablet. Swallow Avmapki capsules whole. Do not chew, cut, or open the capsules. Swallow Fakzynja tablets whole. Do not chew, split, or crush the tablets.

Check with your health care provider before taking any medicines for indigestion, heartburn, or upset stomach while taking Avmapki Fakzynja Co-Pack. If you take antacids, it is important to take your dose of Fakzynja at least two hours before or two hours after you take the antacid. 

You will need to have your eyes checked and get regular blood work before and while taking Avmapki Fakzynja Co-Pack. It is important that you attend all appointments, so your healthcare provider can make sure this treatment is safe for you.

Avmapki Fakzynja Co-Pack can make your skin sensitive to the sun (photosensitivity). Limit your exposure to sunlight. If you need to be in sunlight, wear sunscreen (SPF 30 or higher), a hat, and clothing that covers your skin. If you get a sunburn or any other skin problems while taking this medicine, tell your health care provider right away.

Avmapki Fakzynja Co-Pack may make it hard for you to get pregnant or get your partner pregnant. If you have concerns, talk to your health care provider.

If you could get pregnant, use effective birth control during treatment and for one month after your last dose of Avmapki Fakzynja Co-Pack. If your partner could become pregnant, you should use effective birth control (such as condoms) during treatment and for four months after your last dose of Avmapki Fakzynja Co-Pack. 

What should I tell my health care provider before taking Avmapki Fakzynja Co-Pack?

Tell your health care provider about all of your health conditions and any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. This will help them know if Avmapki Fakzynja Co-Pack is right for you.

In particular, make sure that you discuss any of the following.

Current and Past Health Conditions. Tell your health care provider if you have any of the following.

  • Liver problems
  • Eye or vision problems
  • Severe skin reactions

Other Medicines and Supplements. Avmapki Fakzynja Co-Pack may interact with other medicines and supplements. Before taking Avmapki Fakzynja Co-Pack, tell your health care provider about any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. Pregnancy. Avmapki Fakzynja Co-Pack may cause harm to an unborn baby. Tell your health care provider if you are pregnant or plan to become pregnant. If you become pregnant or get a partner pregnant, contact your health care provider right away.

Breastfeeding. It is not known if Avmapki Fakzynja Co-Pack passes into breast milk. Do not breastfeed while you are taking Avmapki Fakzynja Co-Pack and for two weeks after your last dose. Tell your health care provider if you are breastfeeding or plan to breastfeed.

Interactions

Does Avmapki Fakzynja Co-Pack interact with foods or drinks?

Take Amvapki Fakzynja Co-Pack with food. It is best to avoid grapefruit and grapefruit juice while taking this medicine. The combination may increase the effects of the medicine and raise the risk of side effects. 

It is unknown if drinking alcohol will affect Amvapki Fakzynja Co-Pack, but heavy drinking can slow your immune response and make it harder for your body to fight cancer. 

Does Avmapki Fakzynja Co-Pack interact with other medicines (drug interactions)?

Always tell your health care provider about any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using.

 In particular, make sure that you discuss if you are using any of the following before taking Avmapki Fakzynja Co-Pack.

  • An antacid containing calcium, aluminum, or magnesium
  • An H2 blocker, such as cimetidine (Tagamet), famotidine (Pepcid, Zantac), or nizatidine (Axid)
  • A proton pump inhibitor (PPI), such as esomeprazole (Nexium), lansoprazole (Prevacid), omeprazole (Prilosec), pantoprazole (Protonix), or rabeprazole (Aciphex)
  • ​​St. John’s wort, which is a supplement that may be used for depression
  • Warfarin, which is a blood thinner

Many other medicines may alter the blood levels of Avmapki Fakzynja Co-Pack, and vice versa. Tell your health care provider about all medicines that you take or have recently taken.

What should I do if I accidentally use too much Avmapki Fakzynja Co-Pack?

If you or someone else has used too much Avmapki Fakzynja Co-Pack, get medical help right away, call 911, or contact a Poison Control center at 800-222-1222.

What should I do if I miss a dose of Avmapki Fakzynja Co-Pack?

If you miss a dose of Avmapki Fakzynja Co-Pack, your next steps depend on whether you missed a dose of Avmapki, Fakzynja, or both, as follows.

Avmapki Capsules. If you miss a dose of Avmapki by more than 24 hours, skip the missed dose and only take the next dose at the scheduled time. Do not take double or extra doses. 

Fakzynja Tablets. If you miss a dose of Fakzynja by more than six hours, skip the missed dose and only take the next dose at the scheduled time. Do not take double or extra doses. 

Vomiting. If you throw up after taking Avmapki or Fakzynja, do not take an extra dose. Wait until your next scheduled dose. Do not take double or extra doses.
