Overview: Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine is used to help treat symptoms such as pain, irritation, or frequent peeing that can happen with a urinary tract infection (UTI). It is also used to help relieve discomfort in the urinary tract after medical tests or procedures. It contains five different medicines that work together to help with urinary tract symptoms. Common side effects may include drowsiness, dizziness, blurred vision, or dry mouth. Serious side effects are rare but may include a fast heartbeat or the inability to empty your bladder. This medicine can cause your pee or poop to turn blue to blue-green, which is normal and will go away after you stop taking the medicine.

Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine is commonly used to help treat symptoms such as pain, irritation, or frequent peeing that can happen with a bladder infection (urinary tract infection or UTI). It is also used to help relieve discomfort in the urinary tract after medical tests or procedures.

Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine may also be used for other conditions as determined by your health care provider.

How does methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine work (mechanism of action)?

Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine contains five different medicines that work together to help with urinary tract symptoms. 

  • Methenamine helps kill or stop bacteria from growing in your urinary tract. 
  • Sodium phosphate monobasic helps keep the pee (urine) acidic, which allows methenamine to work properly. 
  • Phenyl salicylate provides mild pain relief. 
  • Methylene blue has mild antiseptic properties to help prevent infection. 
  • Hyoscyamine sulfate relaxes muscles in the bladder to help reduce spasms.

How should I store methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine?

Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine should be stored at room temperature, between 68 F and 77 F (20 C and 25 C). It can be exposed to temperatures between 59 F and 86 F (15 C and 30 C) for shorter periods of time, such as when transporting it. Store in a cool, dry place. Protect from moisture and direct sunlight.

What are the serious side effects of methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine?

While less common, the most serious side effects of methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine are described below, along with what to do if they happen.

Severe Allergic Reactions. Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine may cause allergic reactions, which can be serious. Stop taking this medicine and get help right away if you have any of the following symptoms of a serious allergic reaction.

  • Breathing problems or wheezing
  • Racing heart
  • Fever or general ill feeling
  • Swollen lymph nodes
  • Swelling of the face, lips, mouth, tongue, or throat
  • Trouble swallowing or throat tightness
  • Itching or skin rash
  • Bumps on the skin called hives that can be red, pink, white, or brown depending on your skin tone
  • Nausea or vomiting
  • Dizziness, feeling lightheaded, or fainting
  • Stomach cramps
  • Joint pain

Heart Rate Changes. Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine might make your heart beat faster than normal (tachycardia). Stop taking this medicine and call your health care provider right away if you have a fast or pounding heartbeat.

Inability to Empty Bladder (Urinary Retention). Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine might make it harder to empty your bladder, especially if you already have a blockage. Stop taking this medicine and call your health care provider right away if you are having trouble peeing or cannot empty your bladder.

Breathing Problems. Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine may cause shortness of breath. Stop taking this medicine and contact your health care provider right away or get emergency help if you have trouble breathing or shortness of breath. 

Dizziness or Blurred Vision. Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine can make you dizzy or cause blurry vision. Stop taking this medicine and tell your health care provider right away if you have these side effects.

Allergies to Ingredients. People who are allergic to any of the following should not take methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine.

  • Hyoscyamine

  • Methylene blue

  • Sodium phosphate
  • Urelle
  • Uretron D/S

What should I know about methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine before using it?

Do not take methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine unless it has been prescribed to you by a health care provider. Take it as prescribed.

Do not share methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine with other people, even if they have the same condition as you. It may harm them.

Keep methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine out of the reach of children.

Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine can cause your pee or poop to turn blue to blue-green. This is normal and will go away after you stop taking the medicine.

Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine can affect your alertness or coordination. Do not drive or do other activities that require alertness or coordination until you know how this medicine affects you.

People who are 65 and older and young children can be at a greater risk of some side effects from methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine. Talk to your health care provider about your risks if you are in one of these age groups.

Talk to your pharmacist before taking any over-the-counter (OTC) medicine while you are taking methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine. Taking an antacid at the same time as this medicine may make this medicine not work as well. If you need to take an antacid, separate it from this medicine by one hour. 

What should I tell my health care provider before using methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine?

Tell your health care provider about all of your health conditions and any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. This will help them know if methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine is right for you.

In particular, make sure that you discuss any of the following.

Current and Past Health Conditions. Tell your health care provider if you have any of the following.

Problems With Certain Medicines. Tell your health care provider if you have had any problems or side effects in the past after taking a medicine known as a belladonna alkaloid (such as atropine or scopolamine) or a salicylate (such as aspirin).  

Other Medicines and Supplements. Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine may interact with other medicines and supplements. Before taking methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine, tell your health care provider about any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. Pregnancy. It is not known if or how methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine could affect pregnancy or harm a fetus. Tell your health care provider if you are pregnant or plan to become pregnant.

Breastfeeding. Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine passes into breast milk. Tell your health care provider if you are breastfeeding or plan to breastfeed. Your health care provider will let you know if you should take this medicine while breastfeeding.

Interactions

Does methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine interact with foods or drinks?

There are no known interactions between methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine and foods or drinks.

It is unknown if drinking alcohol will affect methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine. The risk of certain side effects, such as dizziness, drowsiness, or blurred vision, may increase if you drink alcohol while taking this medicine.

Does methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine interact with other medicines (drug interactions)?

Always tell your health care provider about any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using.

In particular, make sure that you discuss if you are using any of the following before taking methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine.

  • Any medicine that affects serotonin in your body, such as a medicine for depression, anxiety, or certain other mental health conditions 
  • A urinary alkalizer, such as potassium citrate, which is a medicine used to reduce the acidity of urine
  • Sodium bicarbonate, which is a medicine used for heartburn and stomach ulcers and is in some over-the-counter (OTC) and prescription medicines, such as Alka-Seltzer, Zegerid, and Zegerid OTC
  • A thiazide diuretic, such as chlorthalidone, hydrochlorothiazide, or indapamide, which is a medicine commonly used to lower blood pressure
  • An antacid containing aluminum, calcium, iron, or magnesium
  • Ketoconazole, which is a medicine used for certain fungal infections
  • A monoamine oxidase inhibitor (MAOI), which is a medicine usually used for depression or Parkinson’s disease
  • An opioid, such as codeine, fentanyl, morphine, or oxycodone, which is a medicine used to treat certain types of pain
  • A sulfa or sulfonamide medicine, such as sulfadiazine silver, sulfamethoxazole, or sulfasalazine
  • Any medicine used to treat diarrhea, such as loperamide (Imodium)

Methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine may affect the absorption of some other medicines taken by mouth. Tell your health care provider about all medicines that you take or have recently taken.

What should I do if I accidentally use too much methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine?

If you or someone else has used too much methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine, get medical help right away, call 911, or contact a Poison Control center at 800-222-1222.

What should I do if I miss a dose of methenamine/sodium phosphate/phenyl salicylate/methylene blue/hyoscyamine?

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and only take the next dose. Do not take double or extra doses.
