Overview: Rybrevant is used to treat certain people with lung cancer. Some common side effects include rash, muscle or joint pain, nausea, vomiting, or tiredness. Serious side effects are less common but may include infusion reactions, and lung, skin, or eye problems. Notify your health care provider right away if you have fever or chills, new or worsening breathing problems, rash or blisters on your skin, or eye dryness, watering, or discharge.

What is Rybrevant used for?

Rybrevant (amivantamab-vmjw) is commonly used for certain types of non-small cell lung cancer

Rybrevant may also be used for other conditions as determined by your health care provider.

How does Rybrevant work (mechanism of action)?

Rybrevant binds to two proteins on cancer cells. This prevents those proteins from sending signals that tell cancer cells to grow and multiply. This may make the cancer shrink in size. Rybrevant also seems to attract certain immune cells to the cancer and helps other immune cells remove important parts of the cancer cell so they do not work properly anymore. Together these things help your immune system find and destroy cancer cells.

Side Effects

What are the most common side effects of Rybrevant?

The most common side effects of Rybrevant are listed below. Tell your health care provider if you have any of these side effects that bother you.

  • Rash
  • Infected skin around the nail
  • Muscle or joint pain
  • Trouble breathing or shortness of breath (dyspnea)
  • Nausea, vomiting, or constipation
  • Tiredness
  • Fluid retention, which can be in your hands, feet, ankles, face, or whole body
  • Mouth sores
  • Cough
  • Low levels of lymphocytes (lymphocytopenia)
  • Changes in your electrolytes, such as phosphate, potassium, and sodium
  • Decreased albumin levels (albumin is an important protein in your body)
  • Increased alkaline phosphatase or gamma-glyamyl transferase levels
  • Increased blood glucose (sugar) levels

What are the serious side effects of Rybrevant?

While less common, the most serious side effects of Rybrevant are described below, along with what to do if they happen.

Severe Allergic Reactions. Rybrevant may cause allergic reactions, which can be serious. Stop using Rybrevant and get help right away if you have any of the following symptoms of a serious allergic reaction.

  • Breathing problems or wheezing
  • Racing heart
  • Fever or general ill feeling
  • Swollen lymph nodes
  • Swelling of the face, lips, mouth, tongue, or throat
  • Trouble swallowing or throat tightness
  • Itching or skin rash
  • Bumps on the skin called hives, which can be red, pink, white, or brown depending on your skin tone
  • Nausea or vomiting
  • Dizziness, feeling lightheaded, or fainting
  • Stomach cramps
  • Joint pain

Infusion Reactions. Rybrevant can cause reactions that can be serious when it is given to you. These are most common the first time you get a dose of Rybrevant. Your health care provider will likely give you other medicines to lower your risk of having an infusion reaction. Your infusion may need to be slowed or stopped if you have these reactions. Tell your health care provider right away if you have any of the following symptoms when you get your infusion.

  • Fever or chills
  • Joint or muscle pain
  • Headache
  • Redness of your face and neck
  • Trouble breathing or wheezing
  • Chest pain or stomach cramps
  • Swelling of the face, lips, mouth, tongue, or throat
  • Hives
  • Nausea or vomiting
  • Dizziness, feeling lightheaded, or fainting
  • Confusion
  • Feeling weak or tired
  • Your tongue, lips, cheeks, or gums feeling numb

Lung Inflammation. Rybrevant can cause inflammation in your lungs, also called interstitial lung disease or pneumonitis. This can be serious or even life-threatening. These symptoms can be similar to your lung cancer symptoms. Get emergency help if you have fever or new or worsening breathing problems, cough, or chest pain. 

Severe Skin Reactions. Rybrevant can cause a severe skin reaction, called toxic epidermal necrolysis (TEN), that can lead to death if not treated. It can also cause dry skin, itching, and a skin rash that looks like acne. You should use skin cream that does not have alcohol in it while you are using this medicine to reduce your risk of skin reactions. Limit your time in the sun while you are using Rybrevant and for two months after your last dose. Use sunscreen and wear a hat and clothes that cover your skin if you need to be in the sunlight. Your health care provider may recommend medicine to prevent or treat any skin reactions you have. If you get a rash, stop using Rybrevant and call your health care provider right away. Get emergency help if you have any of the following symptoms of TEN.

  • Painful red or purple skin that looks burned and peels off
  • Flat red rash or blisters on your skin, mouth, nose, and genitals
  • Red, painful, watery eyes
  • Flu-like symptoms, including fever, chills, and muscle aches

Severe Eye Problems. Rybrevant may cause eye problems, such as dry or itchy eyes, redness, blurry vision, vision problems, or inflammation of the eye (uveitis), eyelid (blepharitis), or cornea (keratitis). If you get any new eye problems while using Rybrevant and you wear contact lenses, you should take them out and use your glasses until you are able to see your eye health care provider about it. Contact your eye health care provider right away if you have any of the following symptoms.

  • Eye redness, itchiness, or pain
  • Swollen or itchy eyelids 
  • Feeling like you have something in your eye
  • Eye dryness, watering, or discharge
  • Vision changes, such as blurry vision or sensitivity to light

Warnings & Precautions

Who should not use Rybrevant?

Allergies to Ingredients. People who are allergic to any of the following should not use Rybrevant.

  • Amivantamab-vmjw
  • Rybrevant 
  • Any of the ingredients in the specific product dispensed

What should I know about Rybrevant before using it?

Rybrevant is usually given into one of your veins as an intravenous (IV) infusion by a health care provider in a hospital or clinic. Your health care provider will likely give you medicines before each dose of Rybrevant to lower your risk of having an infusion-related reaction. See the Side Effects section for more details. These medicines usually include the following.

  • A medicine for fever, such as acetaminophen (Tylenol)
  • An antihistamine, such as chlorpheniramine or diphenhydramine (Benadryl), which is a medicine commonly used to treat allergy symptoms
  • A corticosteroid, such as dexamethasone, methylprednisolone, or prednisone, which is a medicine for certain inflammatory conditions

Rybrevant is often used with other medicines to treat non-small cell lung cancer. There may be other side effects that can happen when medicines are used together, including serious ones. If you are using Rybrevant with any other medicines to treat your cancer, talk to your pharmacist or other health care provider about any other side effects you may be likely to have. 

What should I tell my health care provider before using Rybrevant?

Tell your health care provider about all of your health conditions and any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. This will help them know if Rybrevant is right for you.

In particular, make sure that you discuss any of the following.

Current and Past Health Conditions. Tell your health care provider if you have any of the following.

  • Lung problems (other than your lung cancer)
  • Breathing problems

Pregnancy. Rybrevant may cause harm to a fetus. Your health care provider may need a test to confirm you are not pregnant before you can start using Rybrevant. You should use effective birth control while you are using this medicine and for three months after your last dose. Tell your health care provider right away if you become pregnant or think you may be pregnant while you are using Rybrevant.

Breastfeeding. You should not breastfeed while you are using Rybrevant and for three months after your last dose. It is not known if Rybrevant passes into breast milk. Tell your health care provider if you are breastfeeding or plan to breastfeed.

Does Rybrevant interact with foods or drinks?

There are no known interactions between Rybrevant and foods or drinks.

It is unknown if drinking alcohol will affect Rybrevant, but heavy drinking can slow your immune response and make it harder for your body to fight cancer.

What should I do if I accidentally use too much Rybrevant?

If you or someone else has used too much Rybrevant, get medical help right away, call 911, or contact a Poison Control center at 800-222-1222.

Rybrevant is usually given by a health care provider in a hospital. Your health care provider will make sure you are receiving Rybrevant as scheduled.
